Clinical History:
-
Mass identified in caudal chest for a few years but recently start to cough a lot, coughing situation got better while on medicines but worse again once medicines finished.
-
Findings:
-
There is a moderate alveolar pattern in the left caudal lung lobe and right cranial lung lobe
(craniomedially and caudally). A milder interstitial to alveolar pattern is present in the caudal segment of the left cranial lung lobe. There is partial decreased volume of the left lung lobes.

There is no evidence of pulmonary nodules or masses.

There is a soft tissue attenuating mass in the right caudal thorax. This mass is in continuity with the diaphragm. Post-contrast the vasculature of the liver is in continuity with the caudal thoracic mass indicating that the mass represents a herniated liver lobe (likely right liver lobes).

There are small mineralizations in the renal pelvis and medulla of the right kidney.
-
-
The cardiovascular and mediastinal structures are normal.
-
Conclusion:
-
1. Herniated liver lobe/s.
-

2. Multifocal alveolar pattern.
-
-
3. Calculi in the right kidney.
-
-
Differential diagnosis to consider for the multifocal alveolar pattern includes pneumonia, haemorrhage, atelectasis/ fibrosis or less likely neoplasia. Given the history I would consider pneumonia more likely.
